No order blank, sales slip, bill, invoice, contract, receipt, certificate or document relating in any way to liquor stored in a warehouse, will be approved by the State Liquor Authority unless the following information is stated upon the face thereof, so as to be both conspicuous and readily legible:
(a)the brand name or names of the liquor involved;
(b)the serial number of each and every barrel or cask of liquor in storage;
(c)the date of the original inspection of each and every barrel or cask of liquor in storage;
(d)the exact number of months the liquor has been in storage at the time of solicitation for purchase and sale;
(e)the number of proof gallons contained in each and every barrel or cask at the time of original inspection;
(f)the unit (proof gallons or cases of sealed containers of a specified content) at which the liquor is being sold and the price per unit;
(g)the proof at which each barrel or cask of liquor is to be bottled;
(h)the size of the sealed containers in which the liquor will be bottled;
(i)the number of cases and containers of liquor which will be sold and delivered; and
(j)the total price required to be paid by the retail licensee in order to acquire sole, exclusive and immediate legal title to the liquor offered for sale, inclusive of all charges which shall be itemized as to bottling, labeling, any and all taxes, strip stamps, storage, insurance, freight, delivery and any other charges which the retail licensee will be called upon to pay.
