(a)Selection of vest vendors shall be a determination to be made by the applicant.
(b)The applicant shall insure that such vests comply with the standards adopted by section 6030.3 and the specifications set forth in section 6030.6 of this Part.
(c)The applicant shall certify to the commissioner, in such form and in such manner as he shall require, that any such vest is classified under the standard adopted by section 6030.6 of this Part as capable of protecting the officer to whom such vest is issued to at least the ballistic specifications of the standard issue weapon and ammunition assigned to or authorized for use by the officer.
(d)The applicant shall secure and maintain a permanent record of the manufacturer's certificate of insurance.
(e)The applicant shall determine whether one or two carriers are to be provided for each vest. A second carrier may be required to permit uninterrupted use of the vest while the first carrier is being laundered.
(f)In connection with any application for reimbursement for the purchase of vests, the applicant shall provide, in such form and in such manner as the commissioner shall require, the following information with respect to each vest:
(1)the name of the officer to whom such vest is issued;
(2)the date on which such officer was issued such vest; and
(3)a statement indicating whether the officer receiving such vest has, at any time in the previous 10 years, been provided with any other vest for which State reimbursement was provided under Executive Law, section 837-d or former Labor Law, article 17-a and the date of any such prior reimbursement.
