(a)Each voucher issued, except special promotional vouchers as approved by the commission, must have printed on its face:
(1)the name of the authorized pari-mutuel wagering entity where the voucher was issued;
(2)the unique computer-generated voucher number;
(3)the date and time the voucher was issued;
(4)the number of the ticket-issuing machine; and
(5)the dollar amount of the voucher.
