(a)Employees who are disabled and require special parking considerations due to their condition must take the following steps in order to obtain a special permit:
(1)The employee must complete the "Application for Medical Parking Permit" and the "Employee Health Service Release of Medical Information for Parking Eligibility."
(2)The employee must obtain a physician's statement outlining the disability.
(3)The employee must request his/her agency parking coordinator to arrange for an examination with the Employee Health Service.
(4)The employee must submit the physician's statement to the Employee Health Service at the examination.
(b)The Employee Health Service will then supply copies of their recommendation to the applicant's personnel office and the Bureau of Parking Services. If the Employee Health Service recommends special parking arrangements and the employer agency approves the application, the agency parking coordinator will contact the employee to coordinate the appropriate arrangements.
